SUBMITTING YOUR APPLICATION1. The reimbursement form is only applicable for: - Medicine (not covered by your health insurance), - Routine/emergency dental treatment, and - Prescription glasses
PREPARING YOUR CLAIMS1. Number your receipts clearly and highlight the claimed amount in GREEN fluorescent. 2. Email it with other supporting documents in pdf format.3. Ensure all file names and emails sent are in the format recommended by EAUC.
NON-REIMBURSEMENTS1. Non-prescription medicine2. Cosmetic dental procedures including retainers and braces3. Sunglasses and non-prescription glasses.
