Mountain View High School understands that some members of our community may wish to have a name other than their name assigned at birth printed in the yearbook. We respect your identity and will use the chosen name for a person when the name is the one that best represents that person. All requests for name changes must be submitted by Friday, October 24th.

Note: Respective yearbook staff are committed to do their best to change each incident of your name appearing in the yearbook, but cannot guarantee that all instances will be discovered and changed. This is not a fool-proof process. 

The earlier in the year that this request is placed, the better the odds will be that we will catch more instances of your name to change.
