Everyone wants good health. However, many people do not have habits that contribute to health, well-being,and vitality. Health professionals now describe lifestyle as one of the most important factors affecting health.This lifestyle assessment, adapted from one created by the U.S. Public Health Service, will help you evaluate how well you are doing at managing the factors that strongly affect your present and future health. For each statement choose the answer that best describes your behavior.
Smoker *
Exercise/Fitness
I engage in vigorous exercise (examples include jogging, swimming,brisk walking, cycling) for 20–60 minutes, 3–5 days a week. *
I do exercises to develop muscular strength and endurance(examples include weight lifting, using exercise bands, pushups,abdominal curls) at least twice a week. *

I get recommended age-appropriate medical screening tests(examples include blood pressure and cholesterol checks, Pap tests,mammograms, dental exams), immunizations, and booster shots. *
I get 7–9 hours of sleep most nights. *
I am not sexually active or I have sex with only one mutually faithful partner or I use condoms, and I do not share needles to inject drugs. *
Assessing Your Wellness
Wellness is an active process of becoming aware of and making choices towards a healthy and fulfilling life.  It is more than being free from illness, it is a dynamic process of change and growth. A good or satisfactory condition of existence; a state characterized by health, happiness, and prosperity; welfare.

“Wellness is a state of complete physical, mental, and social well-being, and not merely the absence of disease or infirmity.” – The World Health Organization.
Physical Dimension
Physical wellness relates to maintaining a healthy body and seeking care when needed. Physical health is attained through exercise, eating well, getting enough sleep and paying attention to the signs of illness and getting help when needed.

I maintain a regular schedule of immunizations, physical and dental checkups (including Paptests, and blood pressure and cholesterol checks), and monthly self-exams of breasts/testicles. *

Intellectual Dimension
Intellectual wellness involves having an open mind when you encounter new ideas and continuing to expand your knowledge. It encourages active participation in scholastic, cultural and community activities.

I enjoy attending special lectures, plays, musical performances, museums, galleries, and/or libraries. *

I exhibit fairness and justice in dealing with people. *
I have a network of close friends and/or family. *

But being spiritually well means knowing which resources to use to cope with issues that come up in everyday life.

Environmental well-being promotes interaction with nature and your personal environment. Everyone can have a strong environmental conscious simply by raising their awareness.
I consciously conserve energy (electricity, heat, light, water, etc.) in my place of residenceand work. *

Financial Wellness
Financial Wellness involves the process of learning how to successfully manage financial expenses. Money plays a critical role in our lives and not having enough of it impacts health as well as academic performance. Financial stress is repeatedly found to be a common source of stress, anxiety and fear for college students.
It is easy for me to balance my financial budget. *
I believe it is more important to spend money than to earn it. *
I am confident that I can plan my financial budget. *
I repay loans on a monthly basis according to the payment schedule (if I have any) *
I have enough money to avoid financial emergencies. *
