City of Los Angeles ADA Public Input Survey
The City of Los Angeles is in the process of updating its Citywide Americans with Disabilities Act (ADA) Self-Evaluation and Transition Plan.

As part of that process, the City is asking for your input on the accessibility of City of Los Angeles programs, services, activities, facilities, and information.

Please complete this survey to share your experiences and feedback. Your input will help the City understand what is working well and where accessibility improvements are needed. To download the survey as a PDF, visit disability.lacity.gov/ada-self-evaluation. Translated PDF versions of the survey in multiple languages will also be available on this webpage.

You are also invited to provide your feedback at the City of Los Angeles Commission on Disability’s Public Hearing taking place on September 17, 2026. For more information about how to participate, please visit disability.lacity.gov/cod-hearings.

Accessibility and Language Assistance
The City of Los Angeles provides disability-related accommodations, including auxiliary aids and services, upon request. To request an accommodation, submit the MyLA311 Disability Related Modification or Accommodations Form online at myla311.lacity.gov or dial 311 or 213-473-3231 (voice), or dial 711 for relay services. Requests should be submitted at least five business days in advance of the event to allow sufficient time for coordination. The City will make every effort to accommodate requests received with less notice.
