In consideration of being allowed participation in Hartmann Inline Rink hockey leagues, drop-in, practices, and any other related events and activities ("activities"), I, the undersigned, acknowledge, understand, and agree that:

1. The risk of injury from the activities involved in this program, the activities, and all related activities is significant, including potential for permanent paralysis and death, and while particular rules, equipment, and personnel discipline may reduce the risk, the risk of serious injury continues to exist; and,

2. I KNOWLINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, EVEN IF SUCH RISK ARISES FROM THE NEGLIGENCE OF THE RELEASEES DEFINED BELOW or others, and assume full responsibility for the results of my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however I observe any unusual hazard during my presence or participation, I      will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and my heirs, assigns, personal representatives, agents and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARLMESS THE HARTMANN INLINE RINK, LLC and its members, managers, directors, officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR THE LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM THE ACTION, INACTION, OR NEGLIGENCE OF THE RELEASEES, THE PARTICIPANT, ANOTHER PARTICIPANT , OR OTHERWISE, to the fullest extent permitted by law.
