Waiver and Release of all Claims 

Please read this form carefully. When you sign this form you waive and release all claims for injuries that might be sustained arising out of their use of the facilities and participation in the activities and programs related to the Gen Inspired. 

Acknowledge rules and regulations: 

I have read and understood rules and regulations as posted on the Gen Inspired website and I hereby agree to uphold them. 

Acknowledge risk injury: 

As a participant in the activities or programs related to Gen Inspired, I recognize and acknowledge that 

there are certain risks of physical injury and I agree to assume the full risk of any injuries, including death, damages or loss which may be sustained as a result of participation in activities or programs. 

Waive, Release & Indemnify: 

I hereby waive, release and discharge any and all claims I may have or may acquire against Gen Inspired, its officers, agents, servants and employees as a result of participation, activities and programs; and I agree to indemnify and hold harmless Gen Inspired, its officers, agents, servants and employees from any and all claims resulting from injuries, damages, and losses, including death, sustained while on while participating in Volunteering activities or programs, except for willful misconduct by Gen Inspired or its authorized personnel. 

I have read and fully understand the above Waiver and Release of all Claims Form.

Photographs and Video Consent, Waiver, Indemnity and Release

I hereby allow Gen Inspired and its representatives to take video clips and photographs and to
make recordings of my voice during volunteering activities or meetups or events. I also allow
Gen Inspired to use the Photographs , audio or Video on the website or any promotion activities
for Gen Inspired.

I further agree that I will have no objections if Gen Inspired uses the audio, video, photography
content on their website or shares the content to broadcast on TV channels authorized and
approved by the Gen Inspired Board. I grant permission to Gen Inspired to reproduce, use,
exhibit, display, broadcast and distribute and create derivative works of these images and
recordings in any media now known or later. I also agree that Gen Inspired can publish all
audio, video and photographic content duly inspected by Gen Inspired authorized Event
Coordinators without sharing the same with any particular individual. I also agree that I
will have no legal authority or right to claim any royalties or any other compensation from this
content.
I understand that Gen Inspired is not responsible for other individuals taking videos or
photographs during events.

Community Volunteer Honor Code Oath
As a volunteer of Gen Inspired, I pledge to uphold the principles of honesty, integrity,
respect, teamwork, and compassion in all my community service endeavors.
● Honesty: I affirm that I will honestly and accurately record my volunteering hours
in the log provided. I understand the importance of transparency and will not
falsify or exaggerate my contributions.
● Integrity: I vow to act honestly, transparently, and responsibly, holding myself to
the highest moral standards. I will perform my duties in good faith and report any
wrongdoing or unethical behavior that I may encounter.
● Respect: I pledge to honor the diversity and individuality of every community
member. I will treat others with courtesy and consideration, embracing
differences in background, culture, and beliefs.
● Teamwork: I commit to collaborate with fellow volunteers, staff, and community
members, working together towards our shared goals. I will communicate openly
and support others in our collective efforts to make a positive impact.
● Compassion: I commit to approach each individual with empathy and
understanding, recognizing the unique needs and dignity of those I serve. I will
strive to make a positive impact on the lives of others, offering help and kindness
where it is needed most.
By signing this oath, I affirm my commitment to these principles and acknowledge that
failure to adhere to them may result in my dismissal from my volunteer position.
I undertake this commitment freely and with the full understanding of the
responsibilities and expectations that come with being a volunteer at Gen Inspired.
