The Iowa Thespian Chapter, an affiliate of the Educational Theatre Association, requires that this form be completed for each delegate (students and adults) participating in the 2023 Iowa Thespian Leadership Day  Enter name exactly as it appears on the school registration form submitted by the Troupe Directors.  Forms MUST be submitted by the registration deadline in order for the delegate to participate in the event.

I. RELEASE
The undersigned hereby releases and agrees to indemnify, save and hold harmless the Iowa Thespian Chapter, the International Thespian Society, the Educational Theatre Association, and all respective officers, employees, agents and representatives of the aforementioned entities from and against any and all claims, demands, causes of actions, losses, liabilities, judgments, damages, costs and expenses (including reasonable attorneys’ fees) resulting from the Delegate listed above participating in the  Iowa Thespian Leadership Day. The undersigned shall give each Organizer prompt written notice of any claim or facts or circumstances that might give rise to any claim for indemnification. The undersigned further agrees to be responsible for Delegate while participating in the Iowa Thespian Leadership Day, including any expenses incurred by the Delegate, caused by the Delegate, and/or any personal injuries which may occur to the Delegate.

II. RULES AND REGULATIONS
The undersigned agrees that the Delegate shall abide by the Iowa Thespian Leadership Day security rules and regulations. The undersigned understands that, if the Delegate violates any of the Iowa Thespian Board’s security rules and regulations, the Delegate may be removed from participating in the event, and the undersigned (or other parents and/or legal guardians) may be financially responsible for all necessary costs incurred.

III. PHOTO/VIDEO RELEASE
The undersigned irrevocably consents to being photographed or being recorded by means of video or audio tape recording by the Organizers, or a designated representative of the Organizers. These photographs and/or recordings can be used, without compensation to the undersigned and/or the Delegate, in any public display, publication or media, or website, or in any manner or form, and at any time by the Organizers in promotion of the mission to promote the theatrical arts and have theatre arts recognized in all phases of education. The undersigned releases the Organizers, and their employees, agents, representatives, associates, Board of Directors members, and consultants from any liability in connection with the use of such materials.

All delegates are responsible for abiding by the following safety and security rules for the 2023 Iowa Thespian Leadership Day. Failure to comply with these rules may result in removal from the event, and could impact your troupe's participating at future Iowa Thespian events.

1. I realize that my attendance at the Iowa Thespian Leadership Day is a privilege that may be revoked if I do not follow this Code of Conduct.
2. I will follow ALL etiquette rules during the event:
       - I will participate responsibly and allow all other delegates to participate without undo interference
       - I will respond to presenters in appropriate ways.
       - I will be attentive and cooperative during leadership day activities
3. I will attend all events, and I will support the performers and presenters.
4. I will respect all participants, including students, chaperones, directors and leadership day staff.
5. I will treat all workshop presenters as professionals and will show them courtesy and respect. I realize that presenters have the authority to report my behavior to the leadership day staff for discipline if I am rude, uncooperative, or discourteous.
6. I realize that any student/troupe causing a disturbance and/or not participating in leadership day events may be excluded from future Iowa Thespian events and reported to my school's administrator in charge of discipline.
8. All sessions will be moderated by a member of the event staff and all delegates must follow the moderator's instructions.
