2021 Meal Order Form
**UPDATE: A form must be submitted for EACH CHILD receiving meals!**

Mapleton is continuing to offer meals (breakfast, lunch, and snack) to every child under the age of 18 in the Mapleton school district that is not being educated within a Mapleton building. If you are unable to complete this form, please call Mr. Jim Machin at 419-945-8178.

Orders for the next week of meals must be submitted by Tuesday at 8:00 AM!

Once an order is placed, you do not need to sign up each week as your order will continue. If you need to update or cancel your order, please make sure to either update this order form or call Mr. Jim Machin at 419-945-8178.

**Please note that if you do not pick-up your meals during the designated time, you will not receive that week's order. If arrangements were not made with Mr. Jim Machin prior to pick-up, you will also be removed from the order list and will have to resubmit your order.

Meals will be able to be picked-up every Monday (unless there is no school due to a holiday) between 9:00-11:00 AM at the loading dock located behind the high school. Please make sure to pay attention to the communications that go out (social media posts, Instant Alert texts/emails, and information posted on the district webpage) in case of changes to pick-up dates and/or times.

If your child is in-school, they are also able to receive free breakfast and lunch and do NOT need an order form submitted.
