I understand that I am enrolling for the 2019 Summer Session at Bonita's School of Ballet. I understand that I am responsible for all tuition payments as registered until a Registration Change Form is submitted, minimum 5 days prior to change. *
In teaching the art of dance, it is entirely appropriate for a teacher to put his or her hands on a student to correct the student's posture, the physical line, position of the student's body or part of his or her body, or to help a student hold a position. BSOB does not tolerate any teacher-student contact that is harmful or inappropriate. In enrolling their child in BSOB, parents should acknowledge that it is BSOB practice to correct students with physical contact. *
Tuition payments are due on or before the first day of each month. After the 10th of each month, a $10 late fee will be applied to all late payments. *
All returned checks will incur a $35 fee. *
Students with unpaid tuition may not be allowed to participate in class until fees are paid in full or arrangements have been made. *
Monthly tuition fee will remain the same regardless of the number of absences in the month. Tuition payments for missed classes are not refundable. Students should contact the studio directly to inquire about attending another age appropriate make up class. Please refer to the studio policies for more details. *
