If you want to allow your child to walk home from school, we must have this form signed and returned to the school. Your child will not be allowed to leave campus unaccompanied unless and until this form has been received by the school.
PLEASE NOTE: THIS FORM NEEDS TO BE FILLED OUT BY BOTH GUARDIANS/PARENTS OF THE STUDENTS.

THIS FORM ONLY APPLIES TO THOSE STUDENTS WHO WILL WALK HOME ALONE.
