Hello,

         Thank you for considering Blend On The Water for your dinner party. We can accommodate groups of 8-28 guests within our regular dining areas, with seating at the managers discretion at the table(s) most appropriate for your group size.  We generally offer 2 hour seatings 7 days a week.  

We require a pre-selected menu for any group larger than 7 guests to be served family style to the table with portions commensurate to the number of guests in the party.  We have attached the current menu options for your consideration.  

This menu price starts at $55.00 per person not including Alcoholic Beverages, Tax, or Gratuity.

The Menu is composed of 2 Family Style Appetizers, and 4 Family Style Entrées. We will provide the guest of honor a complimentary family style dessert platter. Add ons will incur supplemental charges. You can choose an Alcohol Package at the end of this form.

Once this form is submitted, our team will get a notification of the submission and will book your event through our reservations portal Open Table. Once the reservation is made here, you will get an email or SMS notification confirming your reservation.

A Credit Card is required to book a large group event. We will ONLY charge your card will if this agreement is violated. (See terms at end of this form)

All guests will share 4 entrées in family style format from the following standard choices:

Dessert includes all of these: Chocolate Cake, 1 Piña Colada Tres Leches, 1 Coconut Flan

Pepsi, Diet Pepsi, Sierra Mist, Ginger Ale, Cranberry Juice, Pineapple Juice, Coffee

* Confirmation of menu and guest count must be submitted one week prior to event.
* You will be charged for the # of guests guaranteed on this form at minimum, regardless of fewer people showing up.  
* If extra guests arrive, all present guests that order an Entrée will be charged for.
* If you surpass your end time, you will be charged $100.00 per ½ hour, per 10 guests.
*.    50% of estimated grand total will be charged as penalty for cancellations or no shows without a 48 hour notice.
* No outside food or beverages permitted.
*      You may bring your own cake, however, there is a Cake cutting fee of $3.00 per person.
* 22% gratuity & sales tax will be added to the final bill and all drink purchases.
*      Fraudulent charges will be contested through legal action.
*      NYC Mandates all indoor diners 12 or older to be fully vaccinated (2 doses).
*      Children 5-12 years old must have at least one COVID Vaccine dose in order to dine in.

Blend on The Water requires a valid credit card to confirm your reservation. Rest assured, we will NOT charge your card at any moment, unless the terms and agreements of this event are violated, or you fail to cancel or show up to your event without proper notice. Please submit the information below
