The Healing Fund for Japanese Canadians offers programs for community education and healing for survivor families of the former Anglican Priest Gordon Goichi Nakayama’s clergy sexual abuse. These healing initiatives are possible through funding from the Anglican Church of Canada as a step in the reconciliation process. If you require any help with this form please contact the Healing Fund committee.
All personal information provided by applicants will be confidential between you and the Healing Fund for Japanese Canadians Project Office. The Anglican Church of Canada and the National Association of Japanese Canadians will not have access to your personal information.
This feedback form will help the Healing Fund for Japanese Canadians assess the level of community need for our Healing Fund projects. Please answer this form to the best of your ability and as soon as you are able to do so.
This form has three sections. Section one asks for your basic personal information. Section two asks questions about survivors and section three asks questions about survivor family members.
The assessment period is from March 2022 to September 2026.
