North Bay Educate Together - Enrolment Form for After-School Activities
2016/2017 Term 2
Term 2 (12 weeks) starts on Friday 17 February 2017. All children need to re-enrol and are NOT automatically guaranteed a place if they were part of an activity in term 1. Please submit the online form no later than 10 February 2017 to join an activity. ALL enrolments must be done through the online form and places are allocated on a first-come-first-served basis.

After-school activities are run by volunteer coordinators, and overseen by the Parent Teacher Association (PTA). There are various activities for children from 1st to 6th class.

IMPORTANT - As in term 1, to improve and simplify administration, payments for after-school activities are being collected directly by the teachers of the activities. Please note that teachers will pass on 75 cents per child per class to the PTA to enable them to run after-school activities and cover the cost associated with the use of the school facilities.

Full payment is due on the day of the first class, otherwise your child may lose the place if the activity is oversubscribed and the place will go to the next child on the waiting list. (If payment is an issue, please contact the school Principal in confidence).

Activities can only go ahead if we have a minimum of 10 students (per activity) enrolled.
If there are any specific requirements for the activity your child will attend, they will be made known to you by the Coordinator.
