MHS Anonymous Reporting Form
Please use this form to report concerns about your safety or the safety of another person here at Medway High School. You may also use this form to report instinces of bullying or harrassment or instances of a violation of civil rights based on race, gender, religion, ethnicity, sexual orientation, disability, or gender identity.
The information you provide will be emailed to a school staff member who will check theri email DURING REGULAR SCHOOL HOURS. If you or someone you know is in IMMEDIATE DANGER, please call 911 or contact a trusted adult or school staff member.

NOTE: You can make this report completely ANONYMOUS by not telling us your name (we won't know who you are). However, it will be easier to help you if you share your name. If you choose to tell us who you are, we will try our hardest to hide your identity.
