Format:
The goal of each ECHO session is to provide a brief overview of a key topic (30 minutes) followed by a 10–15-minute Q&A session. After each expert presentation, ECHO attendees will review a submitted case study to enhance learning and discussion.

Cases are submitted by care teams around our state to help create a better understanding of diverse practice environments. Case discussion is an opportunity for constructive discussion and analysis of state/community resources. Discussion will focus on systems of care, not individual management decisions. Please also remember do not to share any protected health information/PHI in your case submission or presentation.

For educational and quality improvement purposes, we will be maintaining a Data Repository of all submitted case presentation forms. By submitting this form, you are consenting to Georgia Maternal Health ECHO collecting and housing your case presentation form.

By submitting this form, you have acknowledged that Project ECHO case consultations do not create or otherwise establish a provider-patient relationship between an ECHO clinician and any patient whose case is being presented in a teleECHO session.
