This is a unique scholarship that provides an exceptional opportunity for a dental hygiene student to attend the Ensign Academy of Dentistry's 46th Annual Conference on August 11-12, 2023 in Provo, Utah. Funds awarded through the scholarship can be used to reimburse travel expenses incurred by the student to attend the annual conference, or for whatever other financial needs the student may have.
Amount of Annual Conference Scholarship award: Conference registration fees for student (and spouse if applicable) AND $1,000 check payment to be delivered AFTER full conference attendance.
As this scholarship is awarded dependent upon attendance at the conference,

**Attendance at the conference is required for the scholarship to be awarded (further instructions will be provided to recipients).
FOR SCHOLARSHIP ELIGIBILITY, CANDIDATES MUST FIT THE FOLLOWING CRITERIA;
1. Be a current 'Student Member' of the Ensign Academy of Dentistry. There is no charge for student membership, but it is required to be eligible for the scholarship. If you haven't already done so, visit
ensignacademyofdentistry.com/join and follow the prompts to register as a student member of the Academy by April 15th, 2023
2. Have completed at least two semesters of dental hygiene school before conference dates.
3. Have at least one full semester left to complete after attendance at the conference.
3. Have no previous award of Academy "Annual Conference" scholarship.
SCHOLARSHIP APPLICATION PROCEDURE:
1. Fill out the application form below, review for accuracy, and click on "Submit".
2. Request a letter of recommendation from your current hygiene academic advisor, mentor, or ecclesiastical leader. Give them plenty of notice and confirm that they know the deadline of April 15th, 2023. Follow-up with them to make sure they send the letter to
ensigncademyofdentistry@gmail.com.
**It is the applicant's responsibility to confirm that all pieces of the application are received by the Academy by the deadline of April 15th, 2023. Incomplete applications will not be able to be considered for the scholarship.
APPLICATION DEADLINE:
Each of the above items (online application and letter of recommendation) MUST be received no later than April 15th, 2023. Enrollment as a student member in the Ensign Academy of Dentistry must also be completed by April 15th, 2023. If unsure of your membership status e-mail Kristen at

The Scholarship Review Committee will evaluate each application with careful and thoughtful contemplation. Recipients of scholarship awards will be notified by June 15th, 2023.
