Fourth Presbyterian Basketball Registration Form 2017-2018
Basic schedule is as follows: • Sr High: Practices will start mid-October and are once a week depending on coach’s schedules/availability. Games are on Monday nights starting on November 13 – January 29. • Younger Players: Practices will start mid-October and are once a week, typically on Sundays, depending on coach’s schedules/availability. Games are on Saturday mornings starting on November 18 – February 17.
After filling out this online form, please send payment ($65 per child/family max of $150 ) to the Fourth Pres (either send via mail at 703 E Washington St, Greenville, SC 29601 or drop off in the office). Please note on the check "church basketball". Registration and payment is due by Sunday, 9/24 ($15 late fee if received after).
Unfortunately, for those with multiple children, I need you to fill one of these forms out for each child. I know it's a pain, but it's the best way for me to be able to collect all of the necessary data.
