Spring Testing - Outlier Registration and Student Responsibility Agreement
This form is a registration request for students who live more than one hours drive away from the nearest WAVA/ISWA Testing Site and would like to test with their legal resident school district. All requests will be reviewed, and you will be notified of the final decision. If your request is approved, we will contact your legal resident school district and request testing arrangements for you. Testing with your legal resident school district is a privilege and failure to attend scheduled testing may result in a loss of this privilege.
**This is for Spring Exams Only** If you are a high school student who wishes to register for a November or January Retake Exam, please refer to the Fall and Winter Retake Registration Survey available on your school's website.

11th and 12th Grade Students Only:
In addition to any required Smarter Balance ELA, Smarter Balance Mathematics, and End of Course Biology exams, I would also like to attempt an End of Course Mathematics exam as an alternative option to meet my mathematics graduation exam requirement. This option is only available to 11th and 12th grade students:

IMPORTANT: Local schools may choose testing dates that are different from WAVA/ISWA testing dates. If you are approved to test with the local school, you will be required to test on their testing schedule. Spring exams may be scheduled between March - June 2017.

If there are no makeup days available with your legal school district, you will be required to travel to a WAVA/ISWA testing site to complete your exams.
