New Booster Group Application
The PVHS PTO-Booster Club, Inc. (A.K.A. PTO) would like to offer all parent-run clubs at PVHS the opportunity to organize underneath the PTO as a Booster Group. Parent clubs that become a Booster Group under the umbrella of the PTO will maintain their autonomy while taking advantage of the following benefits:

• Share the PTO’s IRS tax exempt status, allowing the Group to accept tax- deductible contributions from donors without having to incur any costs or file IRS paperwork;
• Enjoy coverage under the PTO’s $1 million liability insurance policy, which is mandated by the PV Unified School District. Premiums tend to run about $500 per club.
• Individual clubs are not responsible for filing any tax returns, since the PTO- Booster Club, Inc. files a consolidated annual tax return on behalf of the PTO- Booster Club and the Booster Groups.
• Instead of depositing money in the club account at the PVHS Bookstore, where PO's must be submitted to access funds and delays can occur, Booster Group funds are deposited in a checking account that can be accessed immediately by the Group’s officers.
• Enjoy the use of the PTO’s concession permits, which allow Groups to use the school concession stands at sporting events as a fundraising activity.

In exchange for these benefits, each Booster Group is expected to comply with the Procedures for Parent Booster Groups; following are some things the PTO requires:

• Booster Groups are charged an annual Administrative Fee that helps cover the costs associated with the non-profit status, liability insurance, permits and tax filings. The fee is assessed on a sliding scale based on total annual income.
• One parent representative must be appointed by each Group; this individual is expected to attend all PTO-Booster Club meetings.
• Booster Groups are required to submit annual and monthly financial statements.
• The Group’s bank account must be opened at a bank designated by the PTO, using the PTO tax id number. Each Booster Group will have their own signers, and the PTO Treasurer and President will have online visibility into the activity of each account.
• Booster Groups are required to submit a copy of their Charter to the PTO.

By organizing as a Booster Group underneath the PVHS PTO-Booster Club umbrella and submitting this application, the parents listed (and subsequent officers) agree to abide by the Procedures for Parent Booster Groups. All Booster Groups agree to be bound by the operational standards established by the Bylaws of the PTO-Booster Club, Inc. which are necessary to maintain its IRS 501(c)(3) tax exempt status.

A New Booster Group application must include at least three parent organizers who are all parents of current PVHS students. A President and Treasurer must be identified; these parents become the signers on the booster bank account.
