Membership Application for Howell First Aid & Rescue #1
1Thank you for your interest in volunteering with the Howell Township First Aid and Rescue Squad #1. Volunteering with our organization is an incredibly rewarding experience, however, due to the services we provide; it at times can be stressful and demanding.

Before applying, we ask that you take the time to review the following requirements:
*Answer a minimum of 15 calls per month
*Commit to a seven hour duty crew per week (seven consecutive hours)
*Attend squad meetings that are held on the first and third Wednesday of each month
*Attend training nights that are held on the second and fourth Wednesday of each month
*Serve on a committee
*Adhere to the bylaws and standing operating procedures of the organization
*Enroll in an EMT class within six months (if you are not already an EMT)

***By submitting this form, you are acknowledging that all questions have been answered truthfully. Falsification or untruthful answers will result in denial of your application. The submission of this form also authorizes the Howell Township First Aid and Rescue Squad #1 to conduct a complete background check. All applicants will also submit to a drug test during the application process, and subsequent random tests while a member of the squad.***
