■ Student must be 16 years old before completing the form. (If student is not 16, your form will be dated for your 16th birthday.)
■ The driver's form expires 60 days from the date signed or at the end of the current semester.

Approval for the form requires the student having less than 9 unexcused absences and having passed 6 of 8 courses (or equivalent) in the preceding semester. ■
Once the form is ready, a confirmation email will be sent to the student's email.■ Form will be available for pick-up in the counseling office on school days before school, between classes, at lunch or after school (office hours are 8:00 am to 3:30 pm), not during class time.
■ This form is not needed if permit has already been earned.
Be advised, at the end of each semester we are required to report to the state any students with 9 or more unexcused absences and/or students that have failed more than 2 classes and the student's permit will be revoked.
IF YOU WILL BE TAKING YOUR PERMIT TEST OVER FALL BREAK, YOU MUST REQUEST YOUR FORM BEFORE FRIDAY, 9/27! THE GUIDANCE OFFICE WILL BE CLOSED FROM 3:30 PM ON FRIDAY 9/27 UNTIL 8:00 AM ON MONDAY 10/7.
