Thank you for your interest in voice lessons. Please fill out this form so I have some basic information
about you and we can start our lessons together. We will discuss lesson length and time together and
pick out a time that works for both our schedules. I look forward to having fun with you and helping you learn to sing your best!
Please keep in mind a few studio policies:
Lessons will begin at the time of the appointment. If you are late, the time will not be made up.
If I begin the lesson late, I will make up the time.
24 hour notice is required to cancel or reschedule a lesson or the student will be charged for the lesson.
I expect students to pay for a month of lessons at the first lesson of the month. This payment can be made
by check or cash. If you would prefer to pay weekly please pay by cash.
