Beaver County Sports Hall of Fame Nomination Form

Nominations are due on or before September 1 for consideration for the next banquet, and nominations are kept on file for five years. To be eligible for induction, a candidate must have a reasonable connection to Beaver County, the candidate must have made an outstanding contribution to sports or sport-related activities, and 35 years must have elapsed from the candidate's date of birth.
