This online form includes several registration and permission forms for MSCR Ski & Snowboard Club. Students under the age of 18 need a parent or legal guardian to complete these forms for (or with) them. Students should not sign their own forms unless they are 18 or older.
This form only needs to be submitted once for the 2022 season. Students attending multiple trips do not need to resubmit this portion for each trip.
In addition to this form required by MSCR, Tyrol Basin and Devil's Head each require a waiver form for all participants and a rental agreement form for anyone renting equipment. Those forms must be submitted online. You will receive a separate email for each trip with a link to those forms.
