As part of the Talented and Gifted/Enrichment Program identification process, parental permission is necessary to administer the Slosson Intelligence Test to students entering into 4th grade and 7th grade. (New students may also be subject to testing.) *

Yes, Ms. Grafft may administer the Slosson Intelligence Test to my child. I understand that by providing my consent, it does not guarantee entry into the Talented and Gifted program, as this is an additional piece of data intended to assist with determining program eligibility.

No, my child may not participate in testing at this time. I understand that I may contact Ms. Grafft should this decision change in the future.
