Thank you for your interest in ATLAS! To complete an application, please fill out the following form and send the required supplementary materials to atlas@nmsnewhaven.org. The application must be completed in one sitting, and some questions require your student's response. So that your student may prepare these questions in advance, we have included them here:

1. Describe a time you exhibited one or more of the "5 C's" (curiosity, commitment, creativity, confidence, compassion). (300 word minimum)

2. Please send us a personal piece of work that makes you proud and reflects who you are. Such projects may include, but are not limited to: photos of a finished school report or project (science, social studies, math, ELA, or any other subject welcome!); videos or recordings of performances; a piece of artwork; a link to a blog or other digital project; etc. Please send your choice of work to atlas@nmsnewhaven.org. Indicate the nature of your work and why it is important to you in the response box below.

We recommend that you write the open ended questions in a word processing document first, then copy and paste to the application form. If you prefer to send a hard copy of any of these materials, please send to:
