Absence Reporting Form - JSE
Please complete this form to report your child's absence. If your child was sick, a doctor's note MAY be required to return to school. Doctor notes for absences should be turned in to Campus Secretary Tracy Blair upon your child’s return. If an absence does not meet the criteria for an excused absence or one of the extenuating circumstances specified in Board policy, the absence will be considered unexcused for attendance accounting purposes.
