CHW/P Recognition Awards                              Premios de reconocimiento a CHW/P
For more than a decade, the Adelante Promotores Recognition Awards have celebrated the important work of Promotores and Community Health Workers throughout our County. This year’s awards will recognize two outstanding Promotores/Community Health Workers who have positively impacted their communities. Winners will be awarded at the End of the Year Celebration on Friday, November 7, 2025.

The award will be presented to individuals who, through tangible actions and commitments, have impacted the health and wellness of their community. 

To be considered, nominations must be received by October 31, 2025

Durante más de una década, los Premios de Reconocimiento Adelante Promotores han celebrado el importante papel de los Promotores y Trabajadores Comunitarios de la Salud en todo nuestro Condado. Los premios de este año reconocerán a dos sobresalientes Promotores/Trabajadores Comunitarios de Salud que han impactado positivamente a sus comunidades. Los ganadores serán premiados en la Celebración de Fin de Año el viernes 7 de Noviembre, 2025 

El premio se otorgará a personas que, a través de acciones y compromisos tangibles, hayan influido en la salud y el bienestar de su comunidad.

Para ser consideradas, las nominaciones deben recibirse antes del October 31, 2025

Categorías de nominación
  • The Promotor(a)/CHW must be employed or affiliated as a Promotor(a)/CHW with an agency or organization as an employee or volunteer. Any individual, group, or organization may make nominations. Self-nominations will not be accepted.
  • Nominations for Promotor(a) of the Year require CHW/P nominee to have at least five years of work (paid or volunteer) as a Promotor(a)/CHW.

  • El Promotor(a)/CHW debe estar empleado o afiliado como Promotor(a)/CHW a una agencia u organización como empleado o voluntario.
  • Cualquier persona, grupo u organización puede presentar candidaturas. No se aceptan auto nominaciones.
  • Las nominaciones para Promotor(a) del Año requieren que el CHW/P nominado tenga al menos cinco años de trabajo (remunerado o voluntario) como Promotor(a)/CHW.
