COVID-19 Leave Template Request
According to IRS guidance, eligible employers claiming the credits must retain records and documentation related to and supporting each employee’s leave to substantiate the claim for the credits. I have created online forms for both Office 365 and Google. These forms will allow employees to submit their request(s) and provide documentation without any physical interaction with the office staff. Also, the form can be customized to notify someone when the form has been completed, so it can help managers track requests.

If your company uses Office 365, you should choose Microsoft Forms. By limiting respondents to company users, you can include a file upload question for documentation. If your company does not use Office 365, you should choose Google Forms. Note that if you require documentation be submitted with the Google form, users must log in with a Google account. Work or personal accounts may be used.
