Please note the following and read the guidance on the school website before completing this form. The parent requesting the leave of absence must be responsible for completing the form.
Brakenhale School is not allowed to authorise absence during term time, unless in exceptional circumstances. These do NOT include family holidays. Parents have a duty to ensure their children’s regular attendance at school and failure to do so is an offence under Sect. 444(1) of the Education Act 1996.
Unauthorised absence could result in a penalty notice from the Local Authority.
