Distance Education (DE) Addendum Form: Emergency Declaration Only
This Distance Education (DE) Addendum Form: Emergency Declaration Only is used to propose a course in a DE modality when the nature of that course clearly implies that it be offered in a DE modality only during a College, State, or Nationwide declared crisis.

The DE Addenda Committee (a CoDE subcommittee), in consultation with the instructor(s) of the proposed course, have determined that the course proposed below cannot easily be delivered in a DE modality and still adhere to State, Federal, and Local standards. Therefore, the DE Addenda Subcommittee and CoDE recommend that this course be offered in a DE modality only if there is a College, State, or Nationwide declared emergency in which all courses must be offered remotely for a specified duration.

Separate Course Approval. “If any portion of the instruction in a new or existing course is to be provided through distance education, an addendum to the official course outline of record shall be required.” CA State Title 5 Education Code: § 55202.
