Each student participating the Youth Football and Cheer program is required to complete a Sports Physical EVERY OTHER YEAR. No players or cheerleaders will be allowed to participate in any activities until this form is completed and on file.

If your child DID NOT have a physician complete a Sports Physical Form at a physical examination last year, you will need to schedule a physical and have a sports physical form completed. You may use this WIAA Complete Physical Form or some physicians have their own forms that are also ok to submit.

*Only physicals dated 2024 can be used as alternate year.

If you child DID submit a Sports Physical Form to either the Youth Football Program or Columbus Catholic Schools, simply answer "Yes"
