2025-2026 Kindergarten enrollment packet request
Complete the form below to request an enrollment packet. Please contact Mrs. Bonachea at jrb@whsd.org or 814-255-8725 with any questions regarding enrollment. 

Please fill out a separate request for each student that you are enrolling. 
Children must be 5 years old by September 1, 2025.
You will be required to provide the following documentation to complete each enrollment packet:
1. Proof of birth date (i.e., birth certificate)
2. Immunization records
3. Two proofs of residency within the district (Westmont Borough, Southmont Borough, or Upper Yoder Township) (i.e., deed, lease, current utility bill, property taxes, vehicle registration, driver's license)
Enrollment forms can be dropped off at the Elementary office during business hours.

*You will be contacted to schedule an appointment for a formal enrollment and/or DIAL-4 screening in May.
