Town of Madison Recreation Department Sports Registration Participant Emergency Card, Liability Release, and Photograph Release Form
Thank you for participating in Town of Madison Recreation sports, programs, and activities. In order to continue our programs, we need to incur a small fee of $35 for resident players and $45 for non-resident players. The fee is designed to cover expenses associated with uniforms, equipment and program/league fees. We recognize that these are financially challenging times for families, but small fees help build a self-sustaining Rec. program in Madison. This will help make our program less dependent on our town budget and donations and helps make each program supported by its participants.

Please complete and sign this form as follows:
  • Enter the required participant and parent/guardian information.
  • Enter the required information and sign the Emergency Card.
  • Fully read and sign the Liability Release and Photograph Release. 
  • Submit the form once completed.
  • Participation Fees: you can pay your participation fee by our Square option on our website, cash or checks can be made out to “Town of Madison” with “Fall sports fees” in the memo. 
Remember to click Submit, and complete a separate form for each participant. 

Starting Monday Sept 8, we will practice on Mondays and Thursdays after school 3:30-5:00 pm. Their coach will meet them at school to walk over to Burke Field with them. They will have a quick snack and then start practice.
