The OSA is excited to be working with you on your program! In order for us to best serve you, please read through this form carefully and complete it with as many details as possible.

Here are a few helpful tips to keep in mind:
• Small meetings must have Program Registration Forms submitted at least one week in advance
• Large events (over 40 attendees) must have Program Registration Forms submitted at least 2 weeks in advance
• Budget requests must be submitted 2 weeks in advance
• AV & Facilities needs must be submitted 2 weeks in advance
• We cannot guarantee that your requests will be granted if you fail to meet the deadlines
