CHECK EACH OF THE FOLLOWING ITEMS stating that you have read and agree to the policies and terms. *

Tuition is as follows: $180/semester for one child, or 4 payments of $45 for 1 child or $220/semester for 2+ in the same family, or 4 payments of $55. Tuition is due the first week of class, and every 4 weeks following (Lessons 1, 5, 9, and 13).

I understand that ALL tuition payments are non-refundable. I understand that I will not receive a tuition refund for missed classes or for discontinuing the program before the end of the semester. I understand that if I decide not to participate, my registration fee and materials fee will not be reimbursed. The full 4 months' tuition is due even if I choose to discontinue the program early. In case of unexpected hardships, I will notify the teacher ASAP.

I have reviewed the class calendar and agree to the dates and payment terms on the calendar. I will use the google calendar that has been shared with me for this class.

I agree to read the weekly parent emails, attend class with my child each week, and model good participation for my child during class by participating with excitement and enthusiasm.

I agree to play the semester music album for my child at home or in the car as often as possible, and complete the weekly home fun activities with my child when possible in an effort to maximize the benefits of the program.

We agree to bring all semester materials to class each week in our Sound Beginnings tote bag. Bags will stay with us in the teaching area so we can access materials as needed.

Coats and shoes are to be removed at the door. Hooks are provided in the entryway for hanging personal items.

I will not use my cell phone or other devices during class. I will give my full attention during each activity and will only talk with other parents before and after class.

I understand that classes will begin and end on time. I will arrive 5 minutes before class starts whenever possible so my child can be ready for class when it begins. If we arrive more than 5 minutes early, we will wait for the welcome sign to be placed on the door before knocking or entering.

If we are going to miss class or arrive late, I will notify the teacher BEFORE class starts by sending a text message (preferred), email, or by leaving a voicemail message. I understand Ms. Bethany will not answer phone calls during class.

We agree to stay home when sick.

I will watch for other children when driving and will pull in the circle driveway from the east side, pull forward, and park on the right.

No gum, candy, food, or toys allowed in class -- we will leave these items in our car or in our tote bag.

Non-enrolled siblings and friends may not attend class without Ms. Bethany's permission.

Ms. Bethany works hard to maintain a safe teaching environment, and has never had a problem with accidents; however, the Sound Beginnings program does involve physical movement. By registering in this program, I, the parent or legal guardian, accept full financial liability for any accident or injury that could possibly happen to my child, myself, or any guest accompanying me, while in Bethany Harris’ home or on her property. I release Musik and Me L.L.C., Frank and Bethany Harris, and all their heirs and connections from all liability.

Media Consent and Release *

I, as the parent or guardian of the above student(s), hereby give Bethany Harris and her representatives permission to print, photograph, and record my child for use in audio, video, film, or any other electronic, digital and printed media. (Names of children will not be shared.) I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation on arising from or related to the use of the image. I maintain the right to request the discontinuation of any image. In addition I further release and relieve Bethany Harris and representatives from any liabilities, known or unknown, arising out of the use of such media.

I respectfully request that my child not be printed, photographed, or recorded by representatives of Bethany Harris
