The safety of our students and employees is our priority. In order to prevent the spread of the COVID-19 and reduce the potential risk of exposure to our students and workforce, we are asking everyone to report your child's COVID-19 illness by completing this form. Your participation is important to help us take precautionary measures to protect you, other students, and our staff.
