Thank you for notifying DVC of your child's absence. Please complete the form below. Required fields marked *

If your child misses school for a doctor's appointment, please provide a doctor's note for your child so that their absence can be excused. If they miss 3 or more consecutive school days, please provide a doctor's note on the 4th consecutive day. An absence must be cleared within 5 days. If you don't contact us regarding your child's absence, it will be recorded as unexcused.

If a student leaves campus without getting an Off-Grounds pass or the parent/guardian does not sign them out, the absence will be counted as unexcused. All reported absences will be verified by the DVC Attendance Manager. Please check PowerSchool to verify that all absences have been cleared.

Reports of students leaving early are not accepted online, absolutely no exceptions! Your student must be signed out by a parent/guardian. If a student is 18 years old, a parent/guardian can request a Student Sign-Out form from Pattie, the Attendance Manager.

Tardies are only excused with a medical/dental note provided by the facility. Each note must be submitted to the Attendance Office within 5 school days.
