This form is intended for a student enrolled in HCS and requires a change in status. Examples for a change in status include a student that has paid a fee, changed from BYOD to an ELI device, or signed the ELI agreement after they had marked no originally during online registration.

RETURNING STUDENTS WITH FEES: For Returning Students, all InfoSnap online registration information must be completed and submitted plus owed fees must be paid before a device will be sent to the student.

This form is not for repair requests or for new students. NEW STUDENTS: If you chose to accept an ELI device during online registration, a device will be sent directly to the school for your use. All InfoSnap online registration information must be completed and submitted before students can receive a device.
