Out of District Transportation Opt in Opt out form
 By completing this, you agree to be charged a $300.00 annual fee per family for elementary students or a $150.00 annual fee per family for high school/middle school students (PM routes only). If you choose to opt out, no fee will be assessed. This fee will be included with your textbook rental charges.  

By checking box # 1 and signing below that your student will use # 437 Bus Services, you are acknowledging you will be assessed a transportation fee.

By checking box # 2 and signing below that your student will not use 437 Bus Services, you are acknowledging you will not be assessed a transportation fee.

The Auburn Washburn Transportation Department will choose a pick up and drop off location in district, if not otherwise stated in the alternate pick up and drop off location section.

Auburn-Washburn USD# 437 Transportation Policy states that transported students are entitled to only one designated pick-up and one designated drop-off address per student. The pick-up and drop-off locations may be different, but each must remain constant.

ANY HOME ADDRESS, HOME, WORK, OR CELL NUMBERS MUST BE CHANGED THROUGH THE SCHOOL BEFORE BUS ROUTING CAN BE COMPLETED.
