FLMS Athletic Registration (Girls Soccer, Track and Field) / Registro atlético de FLMS (fútbol femenino, pista y campo)

In order to participate in athletics at FLMS, this form needs to be completed and submitted prior to any practices or competitions. Also, students need a current physical on file in the Main Office and applicable athletic fees need to be paid.

This registration is for Girls Soccer and Track and Field. Both sports are scheduled to start on Monday, 4/19/21.

Yes, the student and the parent/guardian give consent for appropriate medical treatment if he/she gets injured during participation in a FLMS activity. We understand that my student athlete could be injured at any time during participation in athletics at FLMS. We take full responsibility for this liability and costs of potential injury. We give the staff at FLMS our consent to seek appropriate medical treatment for our student. We release the Fort Lupton Middle School and staff from all potential claims and any and all liability, past, present, and future. Hazing Notification / Notificación de novatadas *

We agree with the following statement: The NPMLA and FLMS prohibit bullying, hazing, intimidation or threats. Hazing includes, but is not limited to humiliation tactics, forced social isolation, verbal or emotional abuse, forced or excessive consumption of food or liquids, or any activity that requires a student to engage in illegal activity. We understand that hazing of any type is not permitted during any FLMS activity. We will not engage in any prohibited conduct and further understand that it is our responsibility to immediately report any acts of hazing that we become aware of to a sponsor, counselor, coach, school support staff, or administrator in my school. By signing this acknowledgement, we affirm our responsibility to prevent and report hazing. We also understand that any violation of this could result in school or team consequences which could include dismissal from the activity and/or further disciplinary consequences and/or referral to law enforcement, as appropriate.
