Snellville Performing Arts - Spring 2021 Mini-Semester Classes Registration Form
We are so excited to welcome you to the Snellville Performing Arts family! Our Spring 2021 Mini-Semester will run from March 3rd - April 15th, with each class meeting once a week. Please see below for the specific class days and times.
There is a $25 registration fee for each individual performer and then each class is $60 for each six-week session. Example:
One Class: $60 + $25 registration fee
Two Classes: $120 + $25 registration fee
Three Classes: $180 + $25 registration fee

Once we receive your registration, you will get a confirmation email and invoice within 24 hours! Once your invoice is paid, we will send a class-specific email with information about what to wear and bring, our daily check-in process, etc. Thank you!

PAYMENT INFO & REFUND POLICIES
PAYMENT: Payment can be made in cash, by check (Payable to Snellville Performing Arts, Inc), through Venmo (@SnellvillePerformingArts), or by credit card.
SUBSTANCES & BEHAVIOR POLICY: Snellville Performing Arts has a zero-tolerance policy for weapons, alcohol, drugs, tobacco, and any other illegal substances. If your child brings any of these items to SPA, is caught with these items at SPA, or is under the influence of these items while at SPA, they will be dismissed from our programs. Regarding behavior, if a student is found to be bullying other students, fighting, etc, we will have a discussion with parents and the student will be at risk of being dismissed from the show. If any of the aforementioned items cause a student to be dismissed from our programs, a refund for thus paid amounts will not be issued.
REFUND POLICY: If a student must exit the program for any reason, please let our Executive Director, Stephanie McDonald know as soon as possible. Because this is a short mini-semester, refunds will not be offered once the session has begun.

CONTRACT
This contract agreement is made between Snellville Performing Arts, Inc, the student performer named above, and his/her/their parent/guardian. The contents of the entire registration packet should be discussed with the student and parent/guardian so that all parties understand the commitment made to participate in their selected Fall classes. By signing below, your family understands that:
-Student performers should attend all classes, arriving on time and prepared with appropriate clothing, footwear, and any materials required for the specific class.
-Student performers are expected to adhere to the Snellville Performing Arts Code of Conduct and understand that if they exhibit inappropriate behavior, they may be removed from our program without a refund.
-Student performers are also expected to adhere to any and all regulations and guidelines put in place in regard to the ongoing Covid-19 pandemic, understanding that they may change to remain in line with precautions provided by federal, state, and local government, as well as the World Health Organization and the Center for Disease Control.
-Student performers are minors and while parents, guardians, family, and directors may have high expectations, our greatest expectation is that our students learn as much as they can from our programs and classes while having fun in a safe, creative, and engaging environment.

Hold Harmless Agreement: I understand that with any activity there is a chance of injury or sickness to a person or damage to personal property. Medical Emergency Agreement/Release: In the event of injury or other need, I authorize Snellville Performing Arts to seek medical treatment at the nearest medical facility. Covid-19 Agreement/Release: According to the GCPBSA, businesses, individuals, and other entities cannot be held liable for damages involving a COVID-19 liability claim, unless the claimant can show that the entity’s actions involved “gross negligence, willful and wanton misconduct, reckless infliction of harm, or intentional infliction of harm.” Under Georgia law, there is no liability for an injury or death of an individual entering these premises if such injury or death results from the inherent risks of contracting COVID-19.
