GU Art Galleries: Visitor Registration
Visitors are required to register in advance of their visit in the event that COVID-19 contract tracing becomes necessary. By completing this form, you are acknowledging that you have read and will comply with the GU Art Galleries safety guidelines including:

- If Sick, Stay Home: If you are experiencing symptoms, stay home and get tested.

- Wear A Mask: Surgical-style or other medical-style masks, and/or double-masking are strongly recommended. We can supply you with a surgical mask upon entry.

- Show Us Proof of Vaccination: Visitors must show proof of vaccination along with a government-issued ID or present their University issued GOCard at time of entry. Visitors with medical or religious exemptions must show proof of a negative PCR test taken within the last 7 days.

- Social Distance: Visitors are encouraged to maintain a safe social distance between households or groups. Visitor capacity limits will be enforced to encourage safe social distancing.
