We sincerely appreciate your generosity towards the Big 5-0: Celebrate Health, Celebrate YOU Campaign. Your thoughtful donation will be allocated solely for the expenses of this initiative. We are grateful for your partnership in bringing this project to fruition.

This form is intended for anyone interested in contributing to the campaign monetarily, materially, or providing services. There are 3 ways to participate:

  1. Make a monetary donation

  2. Contribute items

  3. Provide prizes, gifts, services, or discounts to participants in raffles and to the winners of challenges.

Please keep in mind that we require specific items for this campaign, and they must align with our cause. While we appreciate all offers, some items may not be accepted—not out of ingratitude, but because accepting unnecessary items could hinder our efforts.
