Injuries and Accidents：

I am aware that injuries, as well as accidents can occur in any sports, including but not limited to Wushu. Being fully aware of these risks, I hereby affix my signature to give consent to my child/children/myself to partake in any Colorful Sports programs and activities. I will not hold Colorful Sports management, coaches, employees and contractors or its representatives accountable in the event of any injury as a consequence of partaking. In the event of an accident or emergency, I hereby permit Colorful Sports to seek medical treatment for my child/children/myself. I also agree that I shall bear all medical and related costs. Colorful Sports will take the necessary precaution to ensure the safety of all participants. However, it will not be liable for any injury, damage or loss incurred by the participants.

Advertising：
I understand that advertising photos and videos will be taken occasionally, and hereby grant permission to allow my child’s / children’s / my photo or video to be used for advertising purposes.

Terms and Conditions：

All fees are to be paid at the time of registration and made payable via PayNow or other modes of payment.The registration fee of $50(Including a Colorful Sports T-Shirt and pants) and the course fee is non-refundable upon withdrawal by the applicant before or after the commencement of the course.Colorful Sports reserves the right to re-schedule an alternative day or time should the number of participants be insufficient to form a class.
