CHILD DRAMA GROUP APPLICATION
To enrol simply complete and submit the form below for each child. Drama sessions are £5 per session with a drink and snack offered at break time. The £5 fee can be paid online in advance, or at the beginning of each session. Our starting fee is £15 this includes a MAMA T shirt which will be ordered for your child and will be available for collection the week after.  On completion of this form we will email/text you a confirmation of your child's place in our group along with a start date and an invoice of £15 so we can get your Childs T Shirt Ordered. The £15 includes your child's first session and Named MAMA T Shirt. Applications must be made by a parent or guardian of the student.

Please note our drama work shop will start on a Tuesday evening 1st March 2022, at Priory Campus Lundwood and after the Easter break we will move to  Wednesday Evenings at Barugh Green WMC. See dates below before you sign up.

DECLARATIONS
I am aware and agree to the rehearsal/drama evening changing from Tuesday evenings at Priory Campus to Wednesday evenings at Barugh Green WMC. The change to Wednesdays will start on the 27th April (messages and reminders will go out). *
The change of evenings means that you have to agree, otherwise this will affect our ability to complete a contract with you.
