CDHS Community Volunteer Hours Submission Form
This form is intended to replace hard copy written community hours sheets. Once a student has completed a portion of their community hours they are to submit them to the Guidance Office using the form below. This form can be submitted multiple times. Paper copies of hours submitted through this link are NOT required.

It is recommended to submit hours as they are completed and to not wait until the total required are completed.

Information collected through this form will be verified and then the hours applied and recorded in the student's Ontario School Record (OSR).

Students are encouraged to check with Guidance staff prior to completing hours to ensure that they will be approved as appropriate community hours.

All sections of this form must be completed including contact information of the supervisor where hours were completed as hours will be verified.
