Athlete Leadership University Interest Form
Thank you for expressing your interest in atending Athlete Leadership University. This form is to collect contact information, learn what Athlete Leadership University course(s) you are interested in completing, how far you're willing to travel and help you find a Mentor if needed. 

In order to become an Athlete Leader, you must actively participate in at least one sport each year and have no recent history of misbehavior. 


Please note that this is NOT a registration form for Athlete Leadership University. Instead, this allows Special Olympics Pennsylvania to get the word to more athletes. In order to register, you will still need approval from your Team Leader & Regional Executive Director.

If you selected more than one, please indicate which one you would like to be trained in first. Athlete Leaders cannot take on a second or third role until they have completed the minimum requirements of their current role.

What is the furthest you would be willing to travel to attend an Athlete Leadership University session? *
In order to attend Athlete Leadership University, regardless of age and ability, each Athlete Leader must have a committed Mentor to support them before, during and after training. Support can include, but is not limited to communication, transportation, project preparation, etc.

All Mentors, regardless of personal relationship to athlete, must be a Class A volunteer with SOPA. 

If they are not registered as a Class A volunteer, they should start by filling out the volunteer application. Additional instructions on background checks and other required Class A volunteer trainings will be shared once the volunteer applicaiton is completed. 
Do you have a family member, coach, friend, etc. that's NOT a Special Olympics athlete (current, former or potential) that's 18 years of age or older that would be willing and able to serve as your Mentor? *
If you answered "No", if your Team Leader supports you in becoming an Athlete Leader, SOPA will place a listing for a Mentor need on its website and other external volunteer job boards. 

However, please also continue to work with your coaches, Team Leader, Regional Executive Director, family and other spport people to try and identify a Mentor in your community. 

If you answered "Yes", you will be directed to another page to provide your Mentor's name and contact information so they can also receive information as soon as it becomes availalbe.
