Our office only accepts “med pay” claims from the insurance of the car you were in when inured.
• Your car in the accident-Open a claim under your car insurance’s “med pay”.
• You were in a different car-Open a claim under the car you were in’s “med pay”.

We do not accept “third party claims” This means regardless of who’s fault the accident was, we will not send claims to the insurance company of the other car involved in the accident.

The “Med Pay” portion of your policy or the car you were in’s policy will protect you owing unnecessary fees at the end of your care and gives our clinic the best success of prompt payment.


Before we can accept you as a patient under these conditions you will need to have an open claim with your employer or auto insurance. We will need the information requested in the paperwork below.
