This information will help create a Directory of Native American artists residing in the Chicagoland area. The CAICC Arts & Culture Committee will review and verify submissions on a monthly basis. Inclusion in the Directory is complimentary and is not an endorsement of artists or art services. All artists must comply with The Indian Arts and Crafts Act of 1990 (P.L. 101-644) and provide documentation to verify tribal membership.

If you have questions about this form, please contact the CAICC Arts & Culture Committee at nativeartistdirectory@gmail.com. Please send your CIB/CDIB OR an image the front side of your tribal enrollment card OR proof of descendency letter from your tribe, artist bio, portrait, and art sample images to nativeartistdirectory@gmail.com after the completion of this form.
